There are three steps involved in team Registration:
- TEAM CAPTAINs – Complete and submit the form below for Team Registration. Once payment has been received a spot will be reserved and a confirmation sent
- Complete the Team roster electronically and submit to the Webmaster on the Contact Us page. Please submit ASAP, even if they are incomplete. They can be updated and re-submitted to the admin with updates.
- After the Roster has been submitted, notify the team members that they can complete their Individual Waivers electronically. The Online Individual Waiver Form on the Individual Waiver page of the website. Click on REGISTER. Please ensure that the Team Name field accurately reflects the team name that was registered by the Team Captain. Please DO NOT use the Team Registration below for individual waivers.
- The Webmaster will monitor who has completed their waivers. Waivers MUST be completed by Aug 17, 2012. Team captains can request updates on the status of the Waiver completion from the Webmaster on the Contact Us page.
