Online Team Registration for Team Captains

There are three steps involved in team Registration:

  • TEAM CAPTAINs – Complete and submit the form below for Team Registration.  Once payment has been received a spot will be reserved and a confirmation sent

  • Complete the Team roster electronically and submit to the Webmaster on the Contact Us page.  Please submit ASAP, even if they are incomplete.  They can be updated and re-submitted to the admin with updates.

  • After the Roster has been submitted, notify the team members that they can complete their Individual Waivers electronically.  The Online Individual Waiver Form on the Individual Waiver page of the website.  Click on REGISTER.   Please ensure that the Team Name field accurately reflects the team name that was registered by the Team Captain. Please DO NOT use the Team Registration below for individual waivers.

  • The Webmaster will monitor who has completed their waivers.   Waivers MUST be completed by Aug 17, 2012.  Team captains can request updates on the status of the Waiver completion from the Webmaster on the Contact Us page.

Entries and fees received by July 06, 2012 will be entitled to one free practice session. Please send cheque or money order no later than August 17, 2012 payable to: Carleton Place Dragon Boat Festival Committee P.O. Box 153 Carleton Place, Ontario, K7C 3P3. Fees for teams registered prior to Aug 17 are $650 Cdn. (after Aug 17, $700).
NOTE - A fee of $50 applies if a Steersperson/Cox is required, payable with your team registration fee.
By submitting this form, I certify that all competitors on my team are physically fit & trained to swim at least 50 metres wearing light clothing, I also confirm that each competitor has undergone a medical check & that there is no medical reason why he/her should not take part in the program of races notified by the organizers of the Carleton Place Dragon Boat Festival (CPDBF). I hereby expressly agree to abide by all rules & conditions of the organizers & hereby discharge the CPDBF & the Carleton Place Canoe Club, as well as any other individual or organizations connected directly or indirectly with the CPDBF, from any responsibility in the event of injury, death or loss of property incurred during, as a consequence of, or whilst traveling to/from the CPDBF. I further declare that all fees due as notified by the CPDBF Committee in respect of the crew named, will be paid in full when requested & that in the event of any debts incurred by the said team that the above named Dragon Boat team will reimburse the CBDBF Committee in full within 30 days of the end of said Festival.
